How do you calculate PPM?

For a mass-by-mass concentration, 1 mg/kg corresponds numerically to 1 PPM. The underlying basis should still be stated.

Does PPM always mean mg/L?

No. PPM is a ratio, while mg/L is a mass-per-volume unit. They are approximately equivalent only under appropriate density assumptions, commonly for dilute water.
